Model Publication Scheme

This page explains what information NHS Surrey and Sussex ICB makes routinely available and how it can be accessed.

The Scheme is designed to signpost individuals to information we proactively release as and when it becomes available. The Scheme contains seven classes of information - information falling into each of these classes is published on our website.

Our Scheme is a working document. We try to make as much information available as possible; feel free to contact us  if you cannot find what you are looking for.

If, after reading these pages, you are not able to find the information you are looking for please submit a Freedom of Information request . Please include your full name and an address for correspondence; an email address is sufficient. Please be as specific as possible in your request to help us identify the appropriate information. Details of any charges and re-use of information is contained within our Freedom of Information policy.

We publish information under seven categories:

  1. Who we are and what we do — organisational information, structures, locations and contacts.
  2. What we spend and how we spend it — financial information relating to projected and actual income and 
    expenditure, procurement, contracts and financial audit. 
  3. What our priorities are and how we are doing — strategies and plans, performance indicators, audits, inspections and 
    reviews.
  4. How we make decisions — decision making processes and records of decisions.
  5. Our policies and procedures — current written protocols, policies and procedures for delivering our services 
    and responsibilities. 
  6. Lists and registers — information contained only in currently maintained lists and registers.
  7. The services we commission — Information about the services we commission, including leaflets, guidance and 
    newsletters.

1. Who we are and what we do

How we fit into the NHS structure explaining how we fit into either the local or national NHS structure, or both, including information about our roles and responsibilities.

Corporate governance information including details of our board and governing body members and other key personnel.

Senior executives and governing body members including the responsibilities and biographical details of those making strategic and operational decisions about the provision of our commissioned services. 

Gender pay gap reporting. Published at least annually.

Workforce diversity showing the composition of our workforce. Published at least annually.

Lists of and information relating to key organisations with which we work in partnership for the purposes of identifying the relationship between us and these bodies.

Meetings with pharmaceutical companies and other medical suppliers.

Location and contact details for all public-facing departments.

2. What we spend and how we spend it

Financial statements, budgets and variance reports. This information is made available in conjunction with annual accounts.

Sufficient detail will allow the public to see where we are spending money or planning to spend it and the difference between the two.

Publish financial information at least annually and, where practical, provide half yearly or quarterly financial reports. Include revenue budgets and budgets for capital expenditure. 

Provide details of items of expenditure over £25,000, including costs, 
supplier and transaction information (monthly).

Financial audit reports.

Capital programme with information on major plans for capital expenditure, including any public-private partnership contracts.

Staff and board members' allowances and expenses showing allowances and expenses that can be incurred or claimed and detailing the allowances and expenses paid to individual senior staff and governing body members by reference to categories.

Staff pay and grading structures with details of senior staff salaries in bands of £5,000. For all other posts, reports will show levels of pay by salary range and we will also provide the 'pay multiple' — the ratio between the highest paid salary and the median average salary of the whole of our workforce.

Funding including endowment funds and charitable donations.

Trade Union facility time. Published annually.

Procurement and tendering procedures used for the acquisition of goods and services. 

Details of contracts currently being tendered.

List and value of contracts awarded, invitations to tender and procurement 
frameworks that exceed £30,000.

3. What our priorities are and how we are doing

Annual report.

Annual commissioning plan.

Targets, aims and objectives.

Strategic direction document (five-year plan).

Performance against targets (KPI), performance framework, performance management information, quality improvement plans.

Reports by regulatory organisations, for example by the Care Quality Commission and NHS England.

Audit reports delivered at board or governing body level.

Clinical strategy.

Reports on clinical and corporate governance covering service user surveys, data protection impact assessments (in full or summary format) or any other impact assessment, for example health and safety impact assessments, equality impact assessments.

4. How we make decisions

Board papers with agendas, supporting papers and minutes showing where we made decisions about the commissioning of services as well as documents and papers discussed at these meetings. 

Patient and public engagement strategy.

Public consultations, for example, concerning closures or variations of services, with details of consultation exercises and access to the consultation 
papers, or information about where to obtain the papers. 

Internal communications guidance and criteria used for decision making. This covers internal instructions, manuals and guidelines for dealing with your business, where access to this information would assist public understanding of the way you made decisions.

5. Our policies and procedures

Human resources policies including recruitment and employment.

Equality and diversity policies.

Health and safety.

Pay policy statement.

Standing financial procedures or instructions.

Customer service and complaints policies and procedures.

Data protection and duty of confidentiality, records management, Caldicott Guardian.

Estate management.

Charging regimes and policies including charges made for information routinely published, stating what costs we recover, the basis on which we make them and how we calculate them.

6. Lists and registers

Lists of main contractors or suppliers.

Public land and building asset registers showing the locations and key information that is normally recorded on an asset register, along with some other information from capital asset registers.

Information asset register for the Re-use of Public Sector Information Regulations 2015.

CCTV information with the locations of any overt CCTV surveillance cameras operated by or on your behalf.

Register of interests kept by the ICB.

Register of gifts and hospitality provided to board members and senior personnel.

Disclosure logs indicating information that has been provided in response to Freedom of Information Act and Environmental Information Requests.

7. The services we commission

Clinical and non-clinical services provided or commissioned along with those services for which we are entitled to recover a fee together with those fees.

Patient support groups, including bereavement support.

Patient information leaflets, booklets, newsletters, advice and guidance for patients.

Patient experience covering feedback, concerns and complaints.

Corporate communications and media releases.
